CHANGE OF SURNAME (DONE ONLY AT HEAD OFFICE)
Requirements for change of surname:
- Documents must be submitted in person by the applicant
- Original Birth certificate upon application
- Letter from Regional Offices. (as per the Chief code on the applicant’s ID)
- Police Clearance with Finger Prints
- Letter from the Royal Kraal(Umphakatsi) (as per the Chief code on the applicant’s ID)
- An affidavit applicant
- Accompanying affidavit as per the office’s advice depending on each case
- All affidavits should be accompanied with certified copies of ID’s.
- Times(4 cuttings of advertisement)
- Observer(4 cuttings of advertisement)
- Eswatini Printing and Publishing(4 cuttings)
